Step 1 of 3

So we can get in touch

We'll only use this to reach out about your intake.

Pick any that work — we'll do our best to match.

Step 2 of 3

A little about you

Helps us prepare for our conversation. Share what you're comfortable with — none of this is required.

Step 3 of 3

The more you share, the faster we can find the right fit.

We got your message — thank you for reaching out.

We know this may be the most important step you've taken toward recovery — and we'll get back to you as soon as possible to discuss next steps.

Our intake team will contact you by the next business day. We'll talk through your situation, answer questions, and figure out the best next steps together.

In the meantime, if anything comes up, reach out anytime.

We're glad you took this step.
